I arrived in Toronto earlier this week to do an elective, only to find that my laptop had crapped out on me. After extensive (and very painful) work during my off-time while not in the hospital, I finally figured out that my wireless card had died on me. I'm still able to use internet with a cable, but because of this problem, it created some sort of conflict that made it impossible for me to load my computer properly until I reinstalled windows and each component excluding my wireless driver.
Considering this is the second thing that has died in the last few months, I have decided to bite the bullet and get a new laptop. But enough on that.
As I said, for the past week, I've been in Toronto doing an elective. After speaking to the residents here, I should have probably done a "team" elective, where I would have worked on one of the general internal medicine teams, but I actually prefer doing this subspecialty in nephrology, as it's really helped me develop approaches to some of the more common problems like renal failure and electrolyte abnormalities.
Nothing really too exciting during this rotation; I've managed to luck out thus far and read up on the topics that my preceptor grills me on so that I look semi-knowledgeable. The beginning of the week was a little crappy since one of the transplant patients ended up rejecting the transplant and hasn't recovered ever since (he's in ICU still).
Otherwise, the most 'excitement' I had was today, when a nurse told me that she was concerned because one of the patients had a large mass protruding from his lower abdomen. That was a little surprising to me, since I had been there when the patient was admitted and I didn't remember being too concerned about this guy's abdomen. When I examined him, I almost laughed out loud when I realized that the 'concerning mass' was in fact where he had received his kidney transplant. That's almost as bad as when I was asked to see a patient for 'constipation' and to assess to give him some bowel meds, only to find out that he was in the washroom taking a dump when I went to see him. I realize the reason for this mass wouldn't be too obvious to most people, but this was a nephrology nurse. Maybe she was just having a bad day...
